Memorial Donations

After you make a Memorial Donation, please submit this form so we can mail a card!

You may choose to make a one-time or recurring donation in honor of a loved one to our Memorial Fund. Once we receive your payment and the Memorial Donation Form, we will send a card to the person you designate. The card will bear the name of the person in whose memory you make your gift and show your name as the donor. The amount of your contribution will not be mentioned on the card. Your memorial gift helps the Opelousas Area Cerebral Palsy Clinic give services to children with cerebral palsy regardless of race, religion or ability to pay.
